The instructions say to use FL-350 if the parents have an agreement about child support. After both parents complete and sign it, it must be filed and approved by the court. A court case, such as a divorce case, must already be open. It cites Family Code section 4065.
When it is used
Agency cases and case registry
If the local child support agency is involved, a lawyer from that office must also approve and sign the agreement. If it is not involved, each parent must also complete and submit a Child Support Case Registry Form (FL-191) when filing.

Before you file
Common filing problems to check
These checks come from the official form and instructions. They are not a ranking of rejection reasons; the court decides whether a filing meets its requirements.
FL-192 is not attached, although item 16 says it must be attached and is incorporated into the order.
Item 8 (the guideline amount) is blank; the form says you must complete it and item 9 or 10.
A lower agreed amount is used without the statements item 10 requires.
The agency lawyer does not sign when support has been assigned to a county or public assistance is pending.

No. The form says it must be signed by all parties and the court to be effective.
Can a below-guideline agreement be changed later?
The form’s notice says an order below the guideline can be modified without showing a change of circumstances; an order above the guideline needs a change of circumstances.
